Do you want your name and gender in Schoology and MiSiS to reflect what you use? Please have your parent or caregiver sign the below form (your parent/guardian is the applicant unless you are 18) and bring it to the PACS office. Make sure you take a screenshot of your most recent negative covid test results because once we change your gender in the system, you will be assigned a new student ID number and it unfortunately doesn't link to your previous daily pass.
